Can someone tell me why a QRI module that has worked perfectly for 6 months would suddenly go offline and Cortex triggers a warning saying it is in firmware loader mode?

That does seem odd indeed. Can you first try the following in order to establish if it is just that or something else in addition:
Stop the network. Note the ID of the QRI module (e.g. from the module parent object properties menu) and then send the following direct command:
FA90xxyy4400161595 <enter, enter> , where xxyy is the module ID.
This would normally return a module back out of firmware loader mode. So now run the network and see if the module behaves normally and Cortex does not report further issues. If so then the immediate problem is resolved but the root cause not yet determined. If the problem persists then we will need to investigate further.
